Blocked intake, the usual culprit
A ventilation system that was designed correctly on paper can still fail in the field, and the most common reason is the simplest one: intake that looks present from outside isn't actually open behind the vent cover. A soffit vent installed over solid, uncut sheathing lets no air through at all — it's cosmetic, fastened on but not connected to anything behind it. That mistake is invisible from a ladder and from inside the attic both, unless someone specifically checks for airflow at each vent opening rather than assuming a vent cover means a working vent.
Paint is the second most common blocker, and it's usually not deliberate. A full-house repaint that covers soffit vent louvers along with the trim around them can seal off a system that worked fine the year before. The vent still looks like a vent from the ground. It just doesn't pass air anymore.
Insulation is the third, and it's the one most tied to the attic side of the house rather than the exterior. Loose-fill or batt insulation pushed all the way to the eave, with no baffle holding a channel open above it, blocks the exact path the soffit vent was cut to provide. This tends to happen gradually, as insulation gets added or topped off over the years without anyone checking whether the original baffles are still doing their job, rather than as a single mistake made once.
Any one of these blockages defeats a correctly sized system just as effectively as never installing enough intake in the first place. The exhaust side keeps pulling; it just starts pulling from somewhere other than the soffit, since the air has to come from somewhere. Mixing exhaust types
A roof with a ridge vent, a couple of gable vents, and a power fan, all installed at different points in the house's history, looks well ventilated by vent count. In practice, exhaust vents placed too close to each other compete for the same air rather than cooperating, and the closer, lower-resistance path usually wins.
A power fan mounted near a ridge vent is a common version of this. The fan actively pulls air, while a ridge vent works passively off wind and the stack effect. Given a choice between pulling replacement air up from the soffit, through the whole attic, or pulling it straight down through the nearby ridge vent instead, the fan takes the shortcut. That turns part of the ridge vent into an intake for the section of attic nearest the fan, while the rest of the attic gets swept far less than the vent count on the roof would suggest.
Gable vents cause a similar problem even without power behind them, especially on a roof that also has a ridge vent running the same attic. Wind hitting the gable end can push air in one gable vent and pull it back out the other, or out through a nearby section of ridge vent, without that air ever traveling the length of the attic the way a soffit-to-ridge system assumes. The vents aren't broken individually — they're working against each other as a combination.
The fix is rarely adding another vent to compensate for the shortfall. It's usually removing or sealing off whichever exhaust type is competing with the others, so the air has one real path through the attic instead of several partial ones fighting over the same volume.
Bath fans venting into the attic
A bathroom exhaust fan that terminates in the attic instead of running through the roof or a wall to the outside is one of the most damaging ventilation defects a house can have, and one of the least visible. From inside the bathroom, the fan works exactly as expected — it clears the mirror, it sounds right, it moves air. What it's actually doing is dumping warm, moisture-loaded air directly into the attic every time it runs, instead of expelling it from the house entirely.
That moisture lands on the coldest surfaces in the attic, mainly the underside of the roof deck and nearby framing, and condenses there repeatedly, shower after shower, load after load if a laundry vent shares the same mistake. Over a winter that's a steady, self-inflicted moisture source working against whatever the ventilation system and insulation are otherwise doing correctly. This defect hides well because everything downstream of the bathroom looks correct. The duct exists, the fan runs, the mirror clears. Finding it means following the actual duct route through the attic to its true termination point, not just confirming a duct is present, since a duct that ends a few feet past the ceiling — coiled up, disconnected, or simply left open — behaves the same as no venting duct at all.
Rerouting that duct to a real termination point outside is usually a small job on its own, but it's one of the higher-value fixes available on a ventilation call, since it removes a moisture source that no amount of correctly balanced intake and exhaust can fully compensate for.
