Why the dam formed

Icicles hanging along the eave are often the first visible sign, but they're not the same thing as the dam itself. Icicles form from meltwater dripping off an edge that's still draining, at least partially, while a true dam is the ridge of ice blocking that drainage entirely and forcing water back under the shingle line. A roof can have plenty of icicles and no real dam, or a serious dam with barely an icicle to show for it from the ground, so one isn't a reliable stand-in for judging the other.
Roof geometry plays a role in where dams tend to form first. A valley where two roof planes meet concentrates more meltwater than a straight run of eave, and a section of roof over an unheated porch or garage behaves differently from the section directly above heated living space, since the heat loss driving the whole process isn't uniform across a house with mixed conditioning underneath it. A dam that keeps forming in the same spot winter after winter is usually pointing at something specific about that section, not the roof as a whole.
Safe removal versus roof damage
Steam is the method that actually protects the shingles underneath. None of this is a job to improvise from a ladder in the middle of a cold snap. Getting the removal method right, and getting it done before the ice ridge has weeks to grow, is what keeps the roof intact through the process instead of trading one problem for another.
The attic-side fix
Removing an existing dam doesn't stop the next one from forming, because the removal addresses the ice, not the heat loss that built it.
